Which alternative you chose to the incorrect caption? If you understand enough about noun phrase, you would choose D, and that was the correct replacement after all.
Let’s analyze the supposed to be-replaced caption; what’s wrong with it?
A blue Volvo got severe damaged after hit by truck.
  • A blue Volvo: the subject; correct
  • got: the verb; correct since it already happened in past
  • severe damaged: the supposed to be noun phrase or adjective phrase after got; incorrect
  • after hit by truck: adverbial phrase; correct by arrangement
In the sentence, after the verb got we supposed to have noun phrase or past participle so the meanings were akin. The word damaged is already participle, and to modify the verb participle we need an adverb. The word severe that supposed to modify damaged is not an adverb, it is an adjective. To turn it to adverb, you need to derive the word by adding –ly, severely (please recall the derivatives you have learnt in other session). So the first correct sentence is:
But then, the alternatives to choose do not provide the first correct replacement, so let’s try with the second, adding the verb got with noun phrase. To have a proper noun phrase, you surely the noun, right?! You need to change the phrase severe damaged into a noun phrase. In a noun phrase mostly the noun is placed last and followed by some adjectives to modify it. The word damaged in the phrase is not yet a noun, so you need to use derivatives and turn it into damage. 'The' follows the damage is supposed to be an adjective, and the word severe is one. So here we have the second correct sentence that has similar meaning to the caption:

Noun phrase is a phrase that functions as a noun. A noun phrase consists of a noun as the head word and the modifiers that follow before and/or after. The modifiers could be one or more adjective, article, and adverb.
Look, the green words are the modifier, while the red ones are the head word, the noun. If you find difficulties in identifying the noun phrase, please keep this in mind that any set of words that can be replaced by a pronoun is noun phrase. So then you will find that any pronoun is a noun phrase.
